Your worst restaurant experience?

I was just talking to a co-worker about restaurants and he had a horrible time at one of our local restaurants. So it got me wondering about other peoples most horrible experience. I personally cant think of any off the top of my head, but i have had my fare share of incidents with un-cooked food and horrible servers but nothing that could be classified "horrible".
