Self-esteem and body image issues

Where does our self esteem come from?
And why does it get low or high sometimes?
Do you think confident people are secretly insecure or vain people are pathetic?
Does the media have a lasting effect on teenagers into adulthood and why are some people completely unaffected by outside pressure to look a certain way?

I believe that the fashion industry and the media are responsible for a lot of the pressure on young girls and women to conform to a certain body image. You only have to open any women's magazine to see the stick thin images of the models and celebrities like Victoria Beckham. This can destroy any self esteem some young women have when they cannot possibly confirm to that image.
