I'm kind've in a hurry so I'll try to quickly post what comes to mind. I'll expand on the list later if you want more ideas. Listed below in no particular order:
1. Flesh and Blood (Stars Rutger Hauer)
2. Excalibur (Any of them)
3. Gothic (Might be after the time period you are looking for)
4. Tristan and Isolde
5. King Arthur (Stars Kiera Knightly and is a more fact based account..kinda)
6. The Seventh Seal
7. The Name of the Rose (Starring Sean Connery)
8. Henry V (Starring Kenneth Branaugh)
9. The Lion in Winter (There are two version the original and a remake)
10. Shakespeare In Love
11. Ladyhawke
12. Monty Python and the Holy Grail (A classic spoof, probably not what you are looking for but defiently worth a look.)

Nice list! I've seen some but not others so I will mark them down to watch. I definately liked Flesh and Blood (I like Rutger Hauer), Ladyhawk, and most definately the Monty Python movie!

Ever-After --- Cinderalla-esque romance taking place during the Renaissance (DaVinci even has a role), starring Drew Barrymore.
Dragon Heart --- sentimental film, it can really make you cry. Sean Connery does the voice of the dragon; wild.

I wasn't thinking about fantasies, but Dragon Heart was fun. Lots of action, humor, and just enough sentimentality. Loved the dragon.

I'd like to add:
A Knight's Tale --- with Heath Ledger. Storyline basically consists of an impoverished guy lying his way into jousting as a knight; he's accompanied by a few other characters, and eventually falls for a girl of upper-class.

The only specifically medieval movie I can think of is The Reckoning, starring Paul Bettany and Willem Dafoe. A young priest commits a murder and eventually joins a troupe of traveling actors. In a town they come to a woman is about to be executed for the murder of a young boy.
Some of these might do you, though I can't specify your exact periods -- Dangerous Beauty is about a young Venetian woman taught by her mother to be a courtesan. She's eventually tried for witchcraft. Stars Catherine McCormack and Rufus Sewell. Stage Beauty is about an actor who always plays women and his life is disrupted when it's made legal for women to perform on the stage. Claire Danes and Billy Crudup. A bit later than you want--the 17th century, but magnificent settings and costumes. Of course, Shakespears in Love, chronologically earlier.

I'll have to check out the Reckoning. I've seen Dangerous Beauty and I really liked it.
I'm looking for any time period during the medieval or Renaissance. Maybe even before because I liked the newer King Arthur movie.
