i am from the philippines too...maybe this is because they believe that the education and training in the philippines is not good enough...i think they want to be sure that they give license to really skilled people...i am not saying that your cousin is not good enough...i think things are just like this...i think they look down on filipinos...but i believe things should not be this way...and it is our job to show them that we are good enough and that we are not inferior

i think its because US has a much higher standard of education compared to philippines, even your a license doctor there you still need to go back to school here, they need to make sure you have the knowledge for the job.

All I now that if you want to working abroad you will need to study the language first especially TOEFL for country that using english as their mother language. Second, you need to study again in your expertise for example like your cousin there. It because they are some countries especially USA who have a high expectation on certain fields. So you need to going through some tests to make sure that you will be able to achieve their high expectation. Some countries especially big one like USA or Australia are quite restricted about this. My cousin also worked at Australia and he also study about 5 months before he actually work there..

I think they just have a specific set of guidelines. Maybe it is also specific to the type of equipment she will be using and she needs specialized training in that. Did you know Ireland is crying out for doctors, nurses and any health care workers? It is a very good opportunity for people who are looking to work in another country.
