SlashMySearch Sign up ERROR!

Anyone else run into this problem? I went to the website and clicked on the bottom link where you can sign up. It's not working. An error page shows up after I click it showing a long list of warning messages and some e-mail addresses. The same thing happens when I clicked login.
Any suggestions?
