Why Mold?

I was just getting ready to give my child a drink of apple juice that i had made from frozen concentrate not quite 2 days ago and it is 100% pure apple juice. It has been kept in the refridgerator since. But when i went to pour i noticed that it had already started forming mold on the container that is was in. It really isnt that old so why is it molding.
I have noticed this other times but didnt really know when i had made it so i just thought it was old and dumped it out. My koolaid also does this from time to time that i make but it never does it unless it is old.
Could it be my water?
Could it be my container?
Could it be the juice or koolaid?(none are outdated)
Any information and or tips will be very helpful!

It could be a number of things such as you mentioned but I am betting more securely on the time-frame of your containers 'cleanliness longevity' is. I replace the plastic containers every two months but recently discovered keeping plastic jugs any store-bought brand juice is delivered to the public in is not completely free from harmful bacteria that grow along the inner rim of the container itself, especially if the plastic ring that seals it stays on the bottle.
I started putting frozen concentrate and water in a plastic thermos so it stays fresher longer and make sure the lid is tightly sealed, with store bought brand bottles this secure measure is not guaranteed.
To cut down on possible contamination from mold build-up, use dishwasher safe thermoses and not store bought bottles and if can't, at least rotate bottles every two weeks and wash with hot soapy water and air dry for 24 hours.If still concerned about mold buildup, after hot-washing, place some baking soda all along the top rim and neutralize bacterial growth during the drying time before using the bottle again.
