How Hard / Easy is it to transfer colleges?

I was thinking of transferring colleges. This is my last semester of school. I have 5 classes to go til I graduate. I am afraid of screwing up and having to be in college longer than I all ready have set forth. However, I live 140 miles away from my parents, and 250 miles away from my boyfriend. I was thinking of transferring schools. I am currently in a State University and the college I was considering transferring to offers the same major and the majority of the same courses I took. I am spending a lot on rent right now to live alone, and travelling to see anyone I love is costing me tons. I am pretty miserable up here, and I just thought moving closer to every one would make me a lot happier. Even if I HAD to take one more credit in the spring, if I was near family and my boyfriend I'd feel a lot better. If I went to the college I was considering... the 3 hour drive from my boyfriend would turn into 1 hour, and the 2.5 hour drive from my parents would be 45 minutes. I wouldn't mind getting a cute studio to myself instead of this one bedroom with the balcony that I feel is a little too much for me as well. I'd also be 1 hour away from my brother and sister instead of 3 hours. I looked on the site and though I missed the deadline, they said they are still accepting transfer students for most majors still. Has anyone had experience with this?
