obtaining a copy of my medical records

I need to know when I received certain vaccinations and any associated booster shots for job application (teacher)

If you know who your doctor was when you were a child or you had all your records forwarded to your current doctor,you can ask the office for a record of your immunizations. There usually is no fee for just a shot record and it is always a good thing to have. I don't know your age but, if you are fairly young, your local health department as well as the school you graduated from should also have copies of this information. Good luck! (I'm an R.N.)

do you know your childhood doctor? you can also have you titres checked for immunity (although some of your vaxes may have worn off and you wont show immunity any longer) can you file a religeous exception?
