most back pain is self limiting and resolves within 6 weeks with bedrest, stress reduction, and relaxation.the patient is confined to bed on a firm, nonsagging mattress for 2 to 3 days.a bedboard may be used.bathroom privileges may be permitted, but all other out-of bed activities examples answering the phone, child care, general activity due to restlessness are avoided.the patient is positioned to increase lumbar flexion, which reduces compression of the lumbar nerve roots.the head of the bed is elevated to 30 degrees and the patient slightly flexes the knees or assumes a lateral position with knees and hips flexed curled position with a pillow betweenthe knees and the legs and a pillow under the head.A prone position is avoided because it accentuates lordosis.occasionally the patient is hospitalized for active conservative treatment and physical therapy.Intermittent pelvic traction with 15 to 30 pounds of traction is usually prescribed.traction promotes additional lumbar flexion and relaxation of the affected muscles.physical therapy may be prescribed to decrease pain and muscle spasm.therapy may include use of therapuetic cold example ice infrared radiant heat, hot moist pack, ultrasound, diathermy, whirlpool, and traction.appropriate treatment mades are determined by the patients condition.Impaired circulation, diminished sensation and trauma are contraindications for hot packs.whirlpool therapy may be contraindicated for patients with cardiovascular problems because they are not able to tolerate the associate massive peripheral vasodilation.ultrasound produces deep heat, which may increase discomfort because of swelling in the acute stages.it is contraindicated if the patient has cancer or bleeding disorders.

I think it really depends on what is causing the pain and how severe the problem is. I have already gone through about 1 1/2 months of physical therapy (PT) for my serious soft-tissue back injury, and have about 7 more months of PT left to go. I am by no means confined to bed, but I am restricted in some activities and outright banned from doing others, at least for now, until my healing progresses further.

I had a fractured L5 right side supporting bone and I wasnt aware of it so I continued to ice it down and continue to play sports. Due to the stress on the other left side supporting bone of my L5, it also broke, which cause that whole disc (L5) to slip foward an inch. I had surgery on it about a month ago and for the first 2 weeks I was on heavy bedrest with multiple pillows behind my back for support. My position was either on my right or left side with a straigh back on a straight, no elevation bed. Pertaining to your statement, "a lateral position with knees and hips flexed curled position with a pillow between the knees and the legs and a pillow under the head." Thats how I've been sleeping for the past 4 weeks.
