I have heard Heart diseases are related to diets, stress factors and genetics.I am in my mid twenties and my mother already underwent bypass operation last year.My weight has increased like anything over the past 1 year and so my friends and wellwishers are advising me to take preventive measures from very begining. Now frankly speaking I am an absolute food freak and cannot stay without icecreams and chocolates. But now seeing my mother's condition I am thinking seriously to go for a healthy diet...have to seriously think about my addictions to icecreams :)).Can you suggest me some diet chart which will somewhat help me to prevent heart related disease ?

You have heard correct as heart diseases mostly occur due to different and incorrect life styles and habits too along with what you mentioned, things like diet, stress, genetic and heredity factors. Its either one of these factors or a combination of things involved too,and would depend to what youre related to with.
As you have mentioned, your mother is already a heart patient that makes you prone to heart disease too, and your are over weight , reason must be over eating and eating too much of ice creams and sweets. Once you are able to short list the reasons, you are also able to make a list of the things that you need to take care, the preventive measures. Yes, you have to start acting now, before its too late, and you should learn from your mothers mistakes or infact, know that in your case its heredity, plus your own weight problem involved and if you eat so much of sweet, you must get your diabetes checked up too, as all that adds up.
Heart problems can cause high blood pressure too which occurs to many even when the weight problmes there, so in order to take care of all this, you need to exercise whatever little you can, in the starting and then make it to small brisk walks, increasing them to 40-45 daily, and other light weight exercises.
Diet wise you need to cut off or say gradually reduce and then cut off your sweets and ice creams, as they are killers, nothing else, and learn more about, educate yourself more by reading articles and books on what is the right amount of nutrients and fats, carbohydrates that a body needs, and what is in excess. Once you are determined and will to loose weight and follow a controlled diet, no on can stop you then, but firstly you have to make up your mind to get down to it, thats the first thing to do.
You should follow a balanced diet, make a chart with thing thats you know contain too much of fat and carbos, and reduce it to half the intake at first, and later cut them off and replace them with more nutritous food, like sprouts, fruits, vegetables, oats, cereals, soups, less of oil and butter etc, go for more fresh , raw stuff than tinned etc.
