Teachers performance .....how to evaluate?

To evaluate a teacher has been always challenging and mislead. Generally to evaluate the teacher we look at the outcome of his students in exams. I don't think that students performance in exams should be the basis for teachers' evaluation. How could we find weather a teacher is worthful or not? Whether his work is satisfactory or not? Whether he is giving the required output?

I don't think the evaluation of a teacher should depend on student's performance, I do feel that the college has a good way for evaluating teachers. They gave out sheets and let students choose from the choices. And it is straightly about how teacher perform in class, nothing about student's performance.
