As long as you play the game, there was a choice for you on saving or not saving the little girl? so what's your choice on it...
If save, what will happen next?
If not, what will effect to the player?

The end of the game is different, I chosed to save those little girls and sacrifice them only when I didn't had another choice. I only sacrificed them to save my own life. There are some levels where , if you chose to save the little girls , you will be rewarded more than if you chose to kill them.
As others said before , if you chose to kill them , you will gain some "points" that could be used to buy stuff, like weapons ,ammo, and so on. If you chose to save them you still get rewarded but not as many "points" as if you chose to kill them but there are some levels in which you will get other stuff as a reward besides those "points", if you save the little girls.
I manage to finish Bioshock in hard mode and it's my favorite game so far.

I saved them all except one murderous accident. The game ending didn't live up to the power of the opening and throughout really, but overall a grand game indeed.

If you save you will receive 80 ADAMs per girl, if you kill you will receive 160, but if you save 3 you will receive 200 ADAMs and you will see the "Good End" if you save all of them. If you kill all of them you see the "Bad End" Well, the choice is yours...and remember "We made our choices, but in the end, our choices made us"
