How to get a job with bad history?

In the past, I have had a choppy employment history. I called off a lot, quit jobs with no notice, and I didn't stay at jobs very long. On top of all of that, I have large gaps in my employment dates. I don't know the actual dates for sure, and sometimes I have went a year or two of being unemployed. Now that I have things together, my kids no longer need childcare and I have reliable transportation...I am ready to be serious about employment. However, I am having a hard time with this because of my past employment history. How can I fix my resume and applications to work more in my favor?
