I have been using Spybot S&D for almost 2 years now. They recently updated the engine from 1.4 to 1.5. But i too agree it is not the best. I use Symantec Antivirus for virus protection. And there a few instances where spybot had failed to completely recon and remove the threat. Other spyware removers are more troubling with adds or too much reminders or questions than Spybot. I havnt used the microsoft one.
I feel the best thing is to have an AntiSpy, Antivirus and a Firewall and browse using a protected user with all protections keeping up-to-date.

i use AVG free edition, it's always running and runs a complete scan everyday.
about once or twice a month, i'll run a more complete scan with SUPERanti spyware free edition (takes about 3 hours and you should close all windows, the AVG scan you can continue working on your comp.while it's running)
before i shutdown i run CCleaner. if you get some kind of strange bugs you can run something heavy duty like combofix but don't run it unless you know what you're doing, or if you have a pro walk you through it.

The best spyware remover that is free is Spyware Doctor.Before using spyware doctor,i used spyware terminator which was detecting a spyware but not able to remove it.then i installed spyware doctor,it not only detected that spyware but many other spyware was found with it.It removed all the spyware for free.and the dangerous spyware detected by spyware terminated was removed just by restarting my computer.
