If your age on your profile is correct then you are under age and need the written consent of your legal guardian. They also have to sign for your name change to take place. Then you just go to your local government agent and get the forms to fill out, you will have to provide documentation confirming who you are and may have to surrender those documents to ensure no fraud is taking place. Where I am you have to run an add in the local papers declaring your request for name change and if anyone contests it you will have to go before a judge to settle the dispute.
In the case of getting married you go down to your local registry office after the legal ceremony and file for a married name change with copies of your birth certificate, photo id, and marriage license.
After all that starts the task of changing every thing over to the new name.

You can get the forms online. I forget how much they cost. You can also get them from a lawyer. It is not very expensive. I changed my name back to my maiden name when I got divorced, but that was so long ago I do not think it would be relevant now. I am sure that too much has changed by now for my advice to be correct.
Most lawyers will give and estimate for free though, and paralegals are even cheaper than lawyers and should be able to do something like that.

no i dnt think so i dnt know about other states but if you in the usa you dnt have to in lets you married
