When I was 4 and my great-grandmother died my mother told me she went to the sky - to heaven, she died at the early morning so she was taken to the hospital that night so she wasn't there when I went to her apartment, so when my mother told me she went to heaven I tought she literally stepped out of the window and flew to the sky.
I personally would tell a small child that at a certain time, when a human is old he dies and he now exists in another place where he feels no pain. I personally don't believe there is something after death so I wouldn't talk about "heaven", "a good place where he is happy" or anything like that, but I wouldn't want to make the child think that his loved one is gone forever and doesn't have a soul and so he will think the same fate is for him too.
How would you explain death to a small child if you were the one there when he/she asked?

when we were small we were said that our Grandpa has gone to GOD's house, those who go to GOD's house cannot come back.

i would be honest, but put it in layman's terms for a young child to understand.
maybe say she has gone to another world called Heaven. she is no longer in our world. when people die, they go to this other world. they move on elsewhere and have finished their life, what they were meant to do on earth. everyone goes there when their time come, whether old or young, so live life the fullest and you will see her again someday when you finish doing what you are meant to do on earth.
