Might I have dyslexia?

I wonder if I might have dyslexia. I had yet another problem today, which caused me some embarrassment, where I misread a date and ended up asking a rather stupid question. I got the numbers switched around in my head. I just now came home from that and thought, "Oh my gosh, might I have dyslexia???" This might explain why I hear things that sound nonsensical, hear certain sounds wrong off and on, and get the order of items in lists all jumbled up and can't seem to follow directions. It also might explain my difficulty in dancing and why math is so especially difficult for me. Don't get me wrong. I do manage to ace my math classes in college. I just keep finding myself making dumb mistakes over, and over, and over, and then having to go back and correct them. I have absolutely no idea how to go about getting tested for this. Any ideas and information, anybody?
