Cell phones..when are they inappropriate?

We've all done it or fallen victim to it..cell phone ringing in the middle of dinner at a restaurant..when is it considered rude? i have a friend that says if you dont want to be contacted, dont get one..he, mind you, is always on his phone..one day he might need to get it surgically removed..
