Why do Social Services/DOCS keep failing our children?

It's devastating to learn that 100's of 1000's of children and babies are abused and neglected. Most of these cases are known to Social Services/DOCS but yet that fail to protect them. What is wrong with this stupid system throughout the whole world!! Just the thought of a child being abused is enough for me to break down, but learning that 1000's are abused everyday is overwhelming! :-(
