Actually how does exchanging in itrader & other online money exchanging works?

I hear lot of times that people talking about exchanging maybe liberty reserve to paypal or alertpay, how does that work, i mean what is it that they do, do they exchange the accounts or the process only involve the transfer of cash? i'm totally confused on how this exchanging works
