creating my own page layout from scratch what should i do

i dont want to know what sites are out there. i need to know what i can do to make these from scratch. are any of you proficiant in programming your own page? do you often fall into the catigory of using pre made layouts. i would like to get into the competitive market of building them what are some of the tips that you can give me?
