Where can I find a good tutorial for making a customized order form?

I want to make a website where people can order customized jewelry I already have onehttp://lilnono.tripod.com/personalizedgifts but i want to either improve it or make another one. I want to make something where people can pick out colors and designs and text and have it go straight to a shopping cart. Kind of like what you can do at vistaprint. http://www.vistaprint.com

i also forgot to mention some times a college will have students that will help you . you can call one of your local colleges and ask.

you mite have to get a web company help you design your website i have a website called web.com and they do every thing for me really a neat site i think they only charge me 40 dollars a month but i can track who is viewing my site how many people are viewing.
