headers and footers for 300?...help me please

i'm typing my research paper and the headers and footers have to have the last name of the writer and the page number. i knew how to do this at one point, but i don't use it all the time. All I need to know is how to get the page numbers to change with the actually page, ex.(smith 1, smith 2, smith 3). please share your knowledge guys, it'll be greatly appreciated=D thx.
