What should my study pattern be to get a good score in GMAT?

I would really like to know from you people what should my study pattern be to get a good score in GMAT. As in, how much time should I devote myself each day to study and what all subjects should I go through? Also, how much is a good score to get into the top universities? I also, would like to travel the world and experience various cultures, so to get into the top universities in all parts of the world, which subject should I devote my time more?
