Reported Attack Sites: Inject by IFrame

hi,
do you have experience injected by Iframe code on your website?
what is your solution to avoid this problem?
the IFrame always came with this url http://3e0.ru:8080/index.php
it will inserted into ALL index files(whatever files named 'index' or 'main') into all folder.
