As others have said: Be positive. Do not let your negative thoughts run rampant. Start thinking positively. Start thinking about all the things that are right with you. Start thinking about the little goals that you will complete and when you do complete them--praise yourself.
Next you have to calm yourself down. When negative thoughts get a hold of you, they're a difficult thing to get rid of. They also wreak havoc on your mental stability i.e. get depressed. So when you find that they're just wreaking havoc, then get up and move. Do something to distract yourself.
Now regarding your problem areas--you have to make a choice to confront them. Either you cold turkey the bad habits or seek help to get rid of them. But you ultimately have to want to choose to change nothing can make you change--not your fear, significant other, friends, family etc.
One thing you might want to do is keep a journal about your problem areas and see if you can't find a pattern. Like I tend to get lazy too and I notice that I have to write down all my tasks down and then start doing them quickly enough so that I don't start this habit of "oh well I'll do it later etc" So maybe it's when you don't get enough sleep, maybe you're fatigued etc..Or maybe if you have people nagging you with the whole "shape up" spiel that just makes you less motivated to do anything.. Basically find what triggers the laziness, or problems and see if they're fixable.
Make sure that there's nothing wrong with your health. When you're running at optimum level then chances are you'll feel fatigued, tired, sluggish etc. Like maybe certain foods simply zap the energy right out of you. Maybe a vitamin deficiency is making it hard for you to concentrate?
So try to keep a journal to see if there are any patterns such as with health, diet, family, friends, significant other etc.
Hope that helps..

There was a typo, my bad. I meant to say that when you are not running at optimal level then chances are you will run into problems.
Also forgot to add: try a bit exercise. It really helps to release endorphins which help make you happier. It generally helps you in so many ways.

I am sure that in some ways I am like you, but not in that particular way, so the human race is saved, lol. I take almost immediate action to rectify problems, but beyond that I am always planning ahead and anticipating problems so that I can avoid them in the first place. Hubby, who is more like you, laughs at what he calls me "scenarios" but I am a big fan of the "What-if" and have saved his neck more than once.
