from Mylot? Do they pay in a form of a check? If so, what the highest you've ever been paid using this site?

MyLot pay through PayPal. They pay automatically on or about the 15th of the month after that in which you made the minimum payout ($10). In other words, if you made $10 by the last day of September, you would be paid automatically into your PayPal account on or about the 15th of October.
You must, of course, tell them your PayPal email (which may or may not be different from the email address you used to sign up). Do this in Preferences in the Payment Information section.
