Disappointment in a Friend

I'm normally a very vocal person when it comes to sharing my feelings, but when it comes to letting someone know how disappointed I am in their actions and treatment towards me, I'm not quite sure how to tell them. I guess I'm a little stumped and don't want to hurt their feelings, even though they're hurting me. What would you say to this person? What have you said in the past to someone who has disappointed you?
