If you aren't that good with html, there are online help forms that will teach you the code. I can't type out all the codes here it would take hours, and then they wouldn't show. Most likely because its html inside a form. Anyway. If you don't want to do that, simply create tables in microsoft word and asign values. I think if you do it right, you can create the forms you need. when you save save as webpage and you should be done.
