Hiya mrzhang,
I was also thinking that he might have meant to say that it was a twinge instead of a cringe.
Twinges are caused by bad postural position, repetetive strain. This can cause Bursitis which becomes inflamed under the shoulder and around and up the middle of the shoulder and travel down the arm too. Twinge is a sharp pain and it´s caused by inflammation. Can be remedied with anti inflammatory medicines and therapy after. It is however very painful at the beginning and can last months.
It´s known as subdeltoid bursitis. Bursa is a sac of synovial fluid intended to prevent friction between various muscles and tendons.
This is the best way I can describe it as I am not a medical person but have had this injury also. It can feel like your shoulder has dislocated itself but it has´nt it´s just a sensation.

Hiya mrzhang,
Probably means that his shoulder is feeling all bunched up and sore. In other words he is suffering some kind of strain from overwork of some kind. Could be a bad position at work probably not his fault as many jobs are to blame for this kind of injury muscle strains are frecuent at any age now. Women who carry children around all day long have this problem and through hauling shopping bags around on both hands can wreak havoc in the shoulders and the neck. That is what my interpretation would be as I have pulled a muscle in my shoulder too and it feels like it´s "cringing" well it felt like it funny expression but good description.
