MLA and APA format style research paper

I have a research paper which consists on questions about Confidentiality of Allied Health. My question is do anyone know how to set up the research paper in MLA or APA format style. I searched online and looked at samples but it is not that clear to me. Do I have to put the question and under put the answer? I know how to do a works citied page so that I am fine with. Help please!! Thank you.

MLA is mostly used for English, Languages, History, Psychology, and anything having to do with Arts and Social Sciences.
APA is more for Business and science papers and things like that.
Since my major is English, all I ever use is MLA, I have never used APA, so I wouldn't know what to tell you about APA.

MLA is the format of the works cited page, not your paper. Your supposed to cite your sources in MLA format.. At my school we used this site: http://citationmachine.net/
