Need format for letter of recommendation for job

Hi, Reference letter can be given in two formats: 1.General reference letter. ........................To whomsoever it may concern.................... This is to refer Mr/Miss.................. is known to me from....... to till date,who is residing at.............. I have known him/her past_____ years ago.He/she is very intelligent,active,in our area cultural meetings(to state about your positive qualities with an example.you can write anything you like about yourself like he is a meticulous planner etc.) I would not take a step back in recommending him for the_____________ position. I am sure Mr/Miss________ would be an asset to your organization,and would play a key role in taking your company to newer heights. With this I remain, Thanks®ards, xyz. References should be written by the person giving the reference. Otherwise how can they give a true account of your work and yourself as a person? They are just being lazy in my opinion.
