There are few possibilities. First of all check your system for viruses, If there is not virus in your computer, then try to use any other pen drive with your computer. If the same problem faced with the other pen drive then there is a voltage problem your power supply output voltages are not enough to complete the voltage requirement of pen drive. Adjust pen drive on other port too to get more clear result. After these action if your problem still there share it with me again. I try my best to solve your problem by text.

Remove your Mcafee Anti-virus, Go on grisoft.com and download AVG Internet Security then scan your system. Most probably it happens due to virus. Did you checked any other pen drive on your computer or not yet? If not, then use any other pen drive then you go for downloading anti-virus.

I am stumped, I would have said it was a virus but reading the previous response it looks as though you have already checked for them.
If my computer is acting up then I tend to save everything I need on an external hard drive and get the system wiped down to factory settings.
