Why should fat people and smokers get mediare for illness the caused themselves

The over eat, get fat, and get the level of health their habits land them. Just like smokers and their cancers, drunks and sorosis of the liver, etc. Why should these people who dont care about themselves get other people to pay for their self inflicted illnesses? Why should they not insure themselves and cash flow their own treatment?
