Questions about Breastfeeding

I have been feeding my baby for 5 months and we have been doing good on it until the last month, I got a matitis, there was a white head on my left n i p p l e, it hurted like nobody business when he sucks on it and since then the white head is still not going away. I called my midwife, she said just put hot face cloth on it, soak the n i p p l e into the warm water, it will eventually go away itself which it is still there... I finally got an infection, I got really sick so I went in and saw my midwife, she prescribed me some anti-biotics.
Two days ago, I got another problem with my n i p p l e again, I had some very servere burning, stabbing kind of pain on it, I don't even know how I can explain the pain... it was even worser last night, I don't know what it is... I'm still waitin gfor my midwife's call back... do you know anything about itchy, stabbing kind of pain n i p p l e? Does it have to do with breastfeeding?
And I would like to know how long did you breastfeed your baby for? I wonder when I should stop? Does it make a big difference for the baby once switching to the formula??
I really hope some moms who had experiences on breastfeeding or the kind of n i p p l e pain can give me some comments on this discussion, it is so painful right now...
