my adsense or google was disabled... how can it be?

I wonder what will happen if your adsense code is disapproved... Everytime I monetize my blog, it has always been rejected due to my adsense code is disapproved. Is there a way that I can apply again for adsense code? How will I do it? Do I have to make another email? Please help me with this predicament. I want to have additional income with google adsense. Thanks a lot. I just don't know how to do it.
