why is it that doctors & lawyers call what they do as practice? is it mathematical fact that 50% of all docs graduate in the bottom half of their class

I do not know about the statistics you quoted. I remember reading an article in the UK in the early 1980's that based on a research it was calculated on average doctors are only 60% correct. Based on personal experiences I think there is a lot of truth in this statement.
Doctors and lawyers need to practice by having clients to hone and keep their skills.
