How do you make Major Decisions in Life? Practicality VS Desires

In this life, there are lots of decisions that we have to make. One of the major decisions that we have to make is choosing the right career path.
I have a friends who is currently having a difficulty choosing between Practicality over Desire. Let me associate Practicality as Nursing and Desire for Medicine. The story actually goes like this. When my friend was still little, she actually wanted to become a doctor because of her Auntie who was also a doctor. So she studied hard and graduated with flying colors. By college, she was forced by her mother to take up Nursing so that if ever she could not become a doctor, at least, she would still be secured in the future as a nurse. Since she wanted to please her mother so much, she decided to take up Nursing. Indeed, she was able to graduate this year, passed the board exam last July and became a new registered nurse.
A month ago, this friend of mine had a dinner with her relatives from the father's side. She was asked on how she was doing and what's next for her. She told them that she wanted to become a doctor but her relatives told her that she is making a foolish decision since doctors in our place are taking up nursing so that they could go abroad. Aside from that, med School is also expensive would require a lot of studying.
My friend's family is not that rich but she has a sponsor who would help her in her studies. She is actually the kind of person who doesn't believe much in her capabilities when I think that she is really capable of doing it. She is also having a separation anxiety between me and our circle of friends.
How ever will I advice this friend who is currently a lost sheep? She is undecided between practicing her profession as a nurse or pursuing for a higher medical degree. If you were in her situation, which would you choose? or if you were in my situation, how would you advise my friend?
