Please can anyone tell me how to get philhealth pin? Can I do that fast?

Please go to the nearest Philhealth office in your vicinity. Bring a copy of your birth certificate if you are single or the marriage contract if you are married. Include copies of your children's birth certificates, if any. Get a Form called Member Application Form from the counter and complete all the required information if you are a new member. If you are employed, then attach a certificate of employment and they will check the number of contributions that your company has paid in your behalf. You will be issued an identification card with a Philhealth number and a copy of the personal data form wherein your dependents are listed. Take care of the Personal Data Form since this is an important document that will be required by hospitals in case of confinement. SM customer service also accepts application for the Philhealth ID.

YOu have to ask your HR. First, they will give you a form (including ER2) for you to complete. If you're single, bring Parent's valid documents as they are your qualified dependents. I think all single applicants are obligated to declare their parents as their dependents if they are still alive; you can declare any of your nearest kin, otherwise. Let your HR process, next. Your PIN will be given soon, it wouldn't take long, this will depend also on how often you make follow-ups to your HR. Once you got your PIN then, I would advise you to please memorize it if you can as this will enable you to use anytime you needed.

Just let the HR do it for you because you are just new. If you want to process by your own, then try but do not be disappointed if they will ask you for corresponding documents that only your HR can provide. Say for instance, the date of your first contribution, etc.. etc.. This is not my principal opinion but my personal experience. Nonetheless, if you know somebody who is working at your preferred Philhealth office to visit, I guess it may offset.

You will most probably need to go to a hphilhealth office and apply for your philhealth card number. if you are not yet employed, you can apply for yourself. if you are employed, your company will have to provide you with forms you will need to fill up and they will submit this for you. goodluck mspitot.

If you are employed then your company's HR will assist you on this. If you are not you can go to the nearest Philhealth office. We went to a Philhealth office last April to have my data updated (change status, name update and for additional dependent). I will consider the process fast in comparison with other government offices.

You need at least 6 months payments prior to using the Philhealth. And these 6 payments should be in a year meaning if you use Philhealth in December , you must have at least 6 posted contributions from December 2010 to November 2011. Be sure to have this contributions certified by the HR in the company where you work for. Don't forget to bring with you the personal data form. The hospital is going to ask for this document. It is a requirement.
