It depend on the authorization letter in what kind? Who is going to authorized? It just simple acknowledgement form the person whom to authorized that he/she is allowed for something. It is like the person who is authorized represent the person who gives the authority.

Hahaha now I'm getting a little confuse. The authority person is the one who will write the authorization letter for a certain name that he/she is being authorized. Is it right?

your cousin needs to write an authorization and mostly it is accepted when it is hand written. that is what i know. just let her/him write something simple like... to whom it may concern, i hereby authorized my cousin (then your name here) to process my documents in my behalf. your cousin can state the reason why she/he can not process her own documents. then after stating the reason nest paragraph would be... hoping for your kind consideration. respectfully yours, then your cousin will put his/her full name and then sign. she also needs to give you her identification card to prove the signature and you also need to bring your identification card as you process her/his documents. hope this can help.

And may i add too, to let your cousin sign over his printed name. That way, when he presents the authorization letter, there is a basis to compare his signature when he facilitates the documents for and in your behalf. It will also be best that he also bring a valid ID bearing his signature like that in the authorization, in addition to your valid IDs too.

To write an authorization letter, the most important thing is that both, the identity of of the person you are authorizing to make procedures on your name, and yours, are clearly stated, and you specify as precisely as posible, for what documents and processes you are autorizing this person.

actually its only easy to create an authorization letter, and most of the time authorization letter is a short sentence only, because when my father authorized me to transact his SSS,i only wrote in the letter that he is authorizing you to transact.

Authorization letter is actually very simple. All it need is his identification number, his signature and purpose of the letter, authorization you/him to do something on their behalf. Most importantly, both of your identification must be presented upon document processing for verifying purpose. In the letter, phone number can be added in case they need to call to verify. Some places or documents allow authorization of people to do it, while some cant. U have to ask him if it's allowed

If you are the person who grant a permission, I think you need to put your signature and ID card number. However I found this link that might help you: http://officewriting.com/category/authorization-letter/
