In my country we don`t keep icecream around the house as it is done in the US where it seems that many people have big freezers. So I make mine with whole milk, chocolate (I prefer the bitter kind)which I melt with a bit of hot water. I add sugar to taste and maybe a dash of ground cinnamon. If it is not so cold, you may add a couple of ice cubes.

Owlwings is right about the chocolate part, but if you want to make an American style chocolate milkshake, you have to use ice cream. You can start with chocolate ice cream and skip all the rest but the milk. To two or three scoops of ice cream, add about a half cup of milk. If you have a blender, use that on it very quickly so the ice cream doesn't all melt. If you don't have a blender, take a fork and mix the ice cream quickly into the milk. If you don't have chocolate ice cream but you do have ice cream, use chocolate syrup, chocolate drink mix, or melted chocolate.

You can either use a drinking chocolate powder (which doesn't need to be made with hot milk), cocoa powder (which usually needs to be made with a small amount of boiling water before drinking, otherwise it will taste 'gritty') or even a bar of ordinary plain or milk chocolate. If you want to make a milkshake using a bar of chocolate, you will need to melt the chocolate first by breaking it into pieces in a bowl and stirring it with the bowl placed in or over boiling water or by giving it about 30 seconds in the microwave. Once you have melted the chocolate, stir in enough milk to dissolve it and then add the mixture to ice cold milk and whisk or liquidise. Add sugar and any flavourings you like (such as a very little vanilla essence, orange flavouring or mint essence).

You can make chocolate milk shake with chocoloate or coco powder mixed with cold milk, cashewnut, badam, banana, peanut, etc in the blender and have them.

well, I mostly have chocolate milkshake from outside, and rarely do we make at home. At home, we prefer Mango or Banana shake mostly.

You need icecream else it's just not a milkshake now is it ? :) I do this.... Take milk and pour it half way then take massive piece of icecream and cram it in their then a tiny bit of coco poweder and nesquick then shake it up and there you go. Also if you have a piece of chocolate put it in.

Just like what they have said, you can use chocolate powder or cocoa powder then put crushed ice, you can also use condensed milk as sweetener. Vanilla or chocolate ice cream is also a great addition to make the shake yummier!
