Are sonograms always right?

My sister has three boys and expecting her 4th child. The doctor told her that she is having another boy. How accurate are sonograms?

I think that they are mostly right however I did know someone told they were having a boy and they wound up having a girl. They bought everything boyish and painted the room blue. They were so upset.
