Orange is the New Black is big hit on newtflix. Do you think Poussey's death was on an accident?

I feel it was on an accident because, he was just trying to help and stay strong. She died because he weight was on her since she was so small. It did not help the situation when Crazy Eye's was out of control. I would love to hear your thoughts.

I am looking forward to next season. I think it's an accident because the guy doesn't seem very experience and he's harmless just he forgot. It shouldn't have happened, but I'm wondering what will happen to the guard next season.
