It happens mostly in long distance relationships.In some cases,if someone approaches them they just can't remain loyal.Its all about controlling your needs,emotions.

They are often insecure, going thru an abusive relationship, or just plain not Happy but too afraid to stand up and let their partner know what they are looking for.

i think some situations make people susceptible to it, like jobs that are away from family/loved ones, or with lots of fieldwork, or requires more than 8 hours every day so they see their co-workers more and share good and bad times.
