My Mycelia on Coffee Grounds Died

This is my first ever attempt to grow mushroom. I think there are 2 reasons why mycelia died. I didn't sterilize the jar, and I used an almost rotten oyster mushroom stem butts. Didn't find any better ones when I was so excited to make mycelia, so I had no choice but to use it.
I'm fermenting violet rice. When it's ready, I'll sterilze the jar, and use fresh stem butts of King Oysters, White Oysters, Portabello, or Shitake.

@LadyDuck Wow! As simple as that? How I wish I can see some when I visit in the province. How many times were you able to harvest? I'm trying to have a supply of mushroom all year round. I hope my second experiment, using fermented substrate will succeed. If not, I have no choice but to do pasteurization. This is for me the most difficult, but based on my research, it's 100 % guaranteed successful.

I don't like mushrooms, I got sick once from them , won't have them again.

@CookieMonster46 so we're not 100% sure that it's because of mushroom because pizza has so many ingredients.

@wittynet I don't know but it was on a pizza, I could ever eat that type of pizza again,
