Today I helped my daughter did her English assignment. But I am really weak at grammar, I knew she got 100, but can someone explain to about the question no. 2 and 4.
Why do they use 'does Rara' instead of 'is Rara'? Same goes for no. 4.
I would be really thankful for anyone who can explain this.

It's really sad to see such poor English used in the questions to be answered by students. Every single question except the last has a serious fault. Below are what I believe they should be:
1. Where are Sinta, Nina and Rara?
2. Does Rara [have] a new classmate?
3. What will they have in seven days? 'Later' is unnecessary OR What will they have seven days later 'in' is redundant
4. Are they ready for the test? 'Do' is an auxiliary verb and is inappropriate here
5. Is the only question which is correct English.
Your daughter seems to have a better command of English than the teacher who set the test. All her answers are perfect except for #4, which should be 'No, they are not ready for the test', but it looks very much as though that is not her fault! Her handwriting is excellent, too!

I agree with @ScribbledAdNauseum that #2 could be "Is Rara a new classmate?" but it would depend entirely on the text given for comprehension whether Rara is the new classmate or whether another person has recently joined the class and so she has a new classmate!

I also think it should be 'is rara' for number 2

2. Is Rara a new classmate?
4.Are they ready for the test?
Number 2 should be "Is.
Number 4 should be "Are
Number 1 : Where ARE
Number 3: Drop "later" It's not needed.
Number 5 is the only one that makes sense.
I am horrible at explaining things, but I am sure someone will come along to help explain things better.

@Cheyee Yes, they've made mistakes with the way they formed the questions.

@Cheyee That's right. Whoever made the questions didn't use proper grammar... It's funny that a grammar test wasn't written correctly.

Oh! You are not alone, English is my second language, so I'm bad at grammar too.

It's about time you showed up again! How is that beautiful baby? How are you doing, beside being brain sore from helping with homework? Is your husband doing okay? Are your other kids doing okay?

@Cheyee That's nice... I'm glad everyone is doing well. She must be getting big by now.
Everything is going well here. I've been off work since January and enjoying myself.

Good work! It's like Owl Wings said - the problem is with how the questions were written. Good job though!

Fyi, that book called Lembar Kerja Siswa (LKS) it's kind of thin book that contain very short summary of subject and many exercises or assignment and unfortunately many of them ( especially foreign language subject like English, Arabic, and others foreign language ) have basic or fundamental yet fatal mistakes on it. I always spend at least two hours before i go to my class to read it in order to avoid fatal mistakes in what I will teach later
Btw sorry for my bad English, I'm not really good at grammar ( one of my weakness as an English subject teacher )

It looks like you already got answer here so I am not going to give my observation but yes, it should be is and are.
