Forgot to attach it

This is what happened to me at work today.
I forgot to attach the image file in the email. After that happened, I sent back the email earlier with a few additional sentences apologizing for the mistake earlier... of course this time with the file attached.
Have you experienced what I experienced earlier?

Happened to me a couple of times as well
Even to few emails that I received. Instead of pointing it out to them, I would ask the required documents from the PIC by replying to the same emails. The PIC can be a different person from the one that send the email.
Most of the time, the sender would send the documents by replying to the follow up emails that I send after realizing the mistake.

Yes, that's quite a common occurrence at my workplace. In the system we use, there are a lot of steps to complete to do most things so there's a good chance that something could easily be overlooked. Part of the system is to attach documents, but you have to refresh the screen once it's attached. That means that it could still not be there even after you've attached it. Even then, though, the system is so bad that it could decide to stop working halfway through attaching a file and you wouldn't know until you get a message from someone saying there's no file there.

Oh, yes! Forgetting to add an attachment to an email is a common oversight in the workplace, no matter where you live and work in the world, and I think it is most prevalent when the sender's job requires her to multitask or do the job of two or three people instead of one. Of course, this can also happen when the sender is distracted by an interruption, noisy background, or traumatic or stressful personal event that is weighing heavily on her mind.
