How do you interpret this meme?

I wrote earlier about "deadnaming" which is using the original name of a transgender individual who has transitioned and uses a different name. (Like Bruce Jenner is now Caitlyn Jenner so if you still call him Bruce it is deadnaming) What got me thinking about it was this meme. I have known a woman for 20 years mostly online through the parrot community but she lives 3 hours away and another local parrot friend and I visited her a few times over the years. She has 1 grandchild who was born as Lily but, according to her mother, obviously had gender issues from a young age. He now has a different legal name, has had the top surgery, and I think is engaged to a girl. Throughout the transition my online friend has portrayed herself as being a supportive grandmother but she just hasn't been. She repeatedly posted young pictures of "Lily" until I pointed out how inappropriate it was. She has done all manner of crap that I don't know about because I live a few states away. Anyway a couple of days ago she posted the above meme. It says that someone thought they were a man inside a woman's body but then they were born. The person depicted is Chuck Norris noted tough guy. My "friend" tried to play it off as a pregnancy joke. I don't see any which way that is possible. This meme, her posting of it, and her refusal to listen to MANY people telling her that she is wrong hasn't budged her at all. She has dug in and is willing to lose her only child and only grandchild by gaslighting and saying that this meme is about pregnancy. I'm so sad to see it. I understand that she doesn't understand or accept the grandchild's transition after all of these years but there was just no reason to post this on her FB where her family definitely would see it and they have.

The picture hasn't shown yet, at least not on my end. I have a feeling I'm still going to think it's in poor taste when it shows up. She isn't being supportive of her grandson if she refuses to even call him by his legal name.

Wait, now I see it. Yeah, it's still tacky. She's a jerk.

She should love her grandchild as he is. As @spiderdust said, I think that she is a jerk.

I see the meme as meaning that when they were born they came out as who they truly feel that they are.
