Have to answer juror questionnaire

I never responded to the first juror questionnaire that was made out to my old name and address. I came to my mom’s house today to find a second notice. I called the number and asked them if I have to fill it out even though I’ve already gotten juror questionnaires in the past made out to my new name and address and have served. I was told yes.
If you have to change name and address you have to fill out the paper form and mail it in. You cannot do it online.
I filled the form out and was ready to mail it but I need to submit a copy of my jury service before I can do it. I will have to come back to my mom’s house with the proof of service so I can make a copy of it.
This whole thing is ridiculous though. I’m lucky that my old address is my mom’s house. Had it been any other I wouldn’t have even know that a juror questionnaire was sent to me.
I told my mom that we probably can’t leave dad’s juror questionnaire unanswered, even though he is not alive. We should call and inform them of his passing. Otherwise, they will sent another notice.

I updated my name and address on the questionnaire, filled the rest out, and mailed today with proof of service.
