What do you use to remove spam from your e-mail box? Does your ISP tag spam for you? What methods/software work best for you?

I use a free service spamkiller, it's called www.0spam.com, you go there, register with them, and set the site to check your normal email. Then you can configure a fake account on your email client that can clean from spam your real email, so when it will check your real email, it will be clean. If you loose some emails (false positive) you can restore them going on 0spam site. It's very effective and free for only 1 account. I use it for an email I use on newsgroups. I use another email on gmail, and gmail has a good spamfilter!
